Intake Form Packet
If after completing a phone consult we both decide that working together is the next best step, I will send you a welcome letter with a link so you can complete the needed paperwork online. This includes the intake form, consent forms, and assorted policies related to my practice (office policies, general information, communications, and social media). The forms can be filled out, reviewed, signed, and securely stored online. Once you have signed the documents they can be printed for your files or you can return to view them online. I encourage everyone to take notes with any questions or concerns they may have so they can be discussed during the intake appointment. Of course, if there is something that would prevent you from signing the document, please contact me so that we can discuss the matter. The information provided in the intake form will be reviewed during the intake session and we will begin to explore your personalized therapeutic process and how we can best work together. It’s important that the intake form is completed at least 24 hours prior to our intake session so I have time to review it prior to our appointment.
